Following a review by the National Transportation Safety Board, officials say that the Fern Hollow Bridge should have been shut down for years before the collapse.
The review was outlined in a hearing last week, which detailed the widespread corrosion and section loss that caused the bridge to collapse in 2022.
The NTSB also said that city officials repeatedly failed to address critical recommendations that were outlined in inspection reports from 2005 to 2021.
Several commuters were severely injured in the collapse, but fortunately no one died.
